DISCLAIMER, ASSUMPTION OF RISK AND WAIVER:
I, the parent or legal guardian of the player(s) I registered, a minor(s), for myself and on behalf of the player(s), our heirs, assigns and next of kin, acknowledge that participation in soccer necessarily involves travel, play in adverse field conditions, contact with considerable force, and risk of severe, permanent physical injury including bruises, scrapes, strained, sprained or torn muscles, tendons or ligaments, broken bones, dislocation of joints, concussion, brain damage, nerve and spinal cord injury, paralysis and death. For me, and on behalf of the player(s) I registered, our heirs, assigns and next of kin, we willingly and voluntarily accept and assume all such risk.

For myself and on behalf of the player(s) I registered, I further acknowledge that the Crawford AuSable Soccer Association, Inc. (“CASA, Inc.”) is primarily administered by volunteers rather than paid professionals. For myself and on behalf of the player(s) I registered, he/she/they and I willingly and voluntarily agree to comply with the stated and customary terms and conditions for participation and, if he/she/they or I observe any unusual significant concern in his/her/their readiness for participation and/or in the program itself, I will remove him/her/them from participation and bring such concern to the attention of the nearest official immediately and also to the regional commissioner as soon as possible thereafter.

In consideration of accepting the registration and permitting the voluntary participation of the player(s) I registered in its programs, for myself and on behalf of the player(s) I registered, our heirs, assigns and next of kin, I hereby release, discharge and agree to hold harmless CASA, Inc., its employees, volunteers, officials, sponsors and other representatives and any and all owners, lessors, lessees or other persons or entities allowing, permitting or authorizing the use of facilities by CASA, Inc. and the agents, employees, officers and directors of said persons or entities from any and all claims, demands, costs, expenses and compensation arising out of or in any way related to any injury or other damage that may result to said participant(s) or to members of my family or my household or individuals I invite or for whom I am otherwise responsible while participating in or present at any CASA, Inc.–sponsored event, including any physical or other injury caused by the negligence of any person or entity described above.

ACKNOWLEDGMENT AND CONSENT:
For both internal and external use, I acknowledge that CASA, Inc. may compile and use addresses and soccer photographs of the player(s) I register. I consent to such uses and hereby waive all rights to compensation.

PLAYER AND PARENT CODE OF CONDUCT:

Players are expected to:

1. Attend all practices and games; and to call the
   coach if unable to attend a practice or game.
2. Be on time for all practices and games (to the
   best of their ability).
3. Be a team player; no ball or field hogging, pass to other players and play position assigned by coach.
4. Be respectful in words, tone and actions to other players, the coaches, the officials and to any spectators.
5. Show good sportsmanship during practices
   and games, win or lose.
6. Help keep the field clean by not littering.

Parents are expected to:

1. Help their child(ren) realize the importance
   of following the above Player Code of Conduct.
2. Check in with the coach before leaving child(ren)
   at practices or games.
3. Be an example of good sportsmanship to your child(ren) and his or her teammates.
4. Refrain from sideline-coaching your child(ren)
   or other children during practices and games.
5. Remain respectful in words, tone and actions to the players, coaches, officials and to any other spectators.
6. Help keep the field clean by not littering; and helping your child(ren) to do the same
